"Evidence of Insurability (E.O.I.) Required", means that some of the coverages you are applying for will not be approved until you have met certain medical
underwriting standards. The forms provided here are used by the insurers' underwriting department to determine whether you are insurable and therefore qualified
to obtain the insurance product.
