Maximum without evidence of insurability: Evidence of insurability
is required for any increase.
Maximum with evidence of insurability: $300,000

Beneficiary: Clicking on the « Confirm » button
at the end of your enrollment is considered by Medavie
Blue Cross as an electronic signature. You do not have to return a signed
form. However, a beneficiary designation form will be available at the end
of your enrollment for your records.
It is important to complete the evidence of insurability form. To do so, at
the end of your enrollment, click the «Confirm» button
and your evidence of insurability form will show on your screen. Please
print, fill, sign and return this form as indicated in the instructions.
