provides you an overview of the different Basic Employee Life options and
allows you to make your selection.
For more information on the level of coverage including the benefit maximum
and termination of benefit, please refer to the Benefits booklet available
on this site under the «Plan Details» section.
Beneficiary: Clicking on the « Confirm » button
at the end of your enrollment is considered by Medavie
Blue Cross as an electronic signature. You do not have to return a signed
form. However, a beneficiary designation form will be available at the end
of your enrollment for your records.
It is important to complete the evidence of insurability form if required.
To do so, at the end of your enrollment, click the «Confirm»
button and your evidence of insurability form will show on your screen.
Please print, fill, sign and return this form as indicated in the
instructions.
