Maximum without evidence of insurability: Evidence of insurability is required for any increase
Maximum with evidence of insurability: $500,000 (combined maximum of $2,000,000 including Basic Life Insurance - Employee)
Termination of benefit: Your 70th birthday or retirement if earlier
It is important to complete the beneficiary designation and evidence of insurability form (if applicable). To do so, at the end of your enrollment, click the «Confirm» button and your beneficiary designation and evidence of insurability forms will show on your screen. Please print, fill, sign and return these forms as indicated in the instructions.
