Amount of insurance for the children: units of $5,000
Maximum: $50,000
Termination of benefit: termination of employment, retirement, age 65 of participant or child age (21 years of age (child age if is not considered as a student) or 26 years of age (child age if is considered as a student))
At the end of the enrollment after you have hit the «Confirm » button you will be provided the required evidence of insurability forms. You will need to complete, sign and return these forms to the address indicated in the instructions.
