Select Your Long Term Disability Coverage (LTD 40)

This page provides you with the opportunity to review the various Long Term Disability options and choose one. If you click on the coverage descriptions underlined in blue you will be provided with a brief description about that option. Detailed information is provided in "Plan Details".

The selection of some options will require evidence of insurability (EOI). At the end of the enrollment process, after you hit the "Confirm" button, you will be provided an opportunity to print out the required EOI form. The form is required by the insurance company to determine whether you satisfy their health requirements. While awaiting the decision you will automatically be enrolled in an option that does not require EOI. Once the decision has been received your plan will be brought up to date and the approved coverage will be effective from that time forward.
