Your
Choices Benefits Plan offers you the opportunity to purchase Critical
Illness Insurance. The plan is designed to provide you with a pre-determined Lump Sum Payment from $10,000 to
$100,000 should you be diagnosed with one of the covered conditions.
After hire, if you elect coverage for the first time, or increase your level of coverage, you will be required to provide evidence of good health satsifactory to the Insurer.
At the end of your enrollment session (after you click the "Confirm" Button) you will be prompted to print the required Evidence of Insuribility (EOI) forms. Complete the form and send directly to the Insurer as instructed.

While waiting the approval the system will automatically enroll you in an option that does not require evidence of insurability. Once your approval has been received, you will be enrolled in the plan effective from the date of insurer approval.
