Through the Choices Benefit Plan, you automatically receive AD&D Insurance
coverage equal to Two (2) times Annual Salary (annual salary & target bonus at Jan. 1st to a maximum of $1 million)
Optional AD&D Insurance
In addition to the Basic Employee Life Insurance the Choices Benefit Plan
offers you the ability to select additional coverage from
$10,000 to $250,000 of coverage in units of $10,000.
After hire, if you elect insurance or increase your levels of insurance, you will be required to provide evidence of good health satisfactory to the insurer.
At the end
of the enrollment after you have hit the « Confirm » button you will be
provided an opportunity to print out the required Evidence of Insurability (EOI) forms.
