If you are a new hire, Evidence of Insurability (EOI) is required on any optional life amount over $80,000. If you elect an amount over $80,000, please print and complete the Canada Life Medical
questionnaire at the end of your enrolment and send to Canada Life as per the instructions.
While awaiting the approval the site will automatically enroll you in an option that does not require EOI. Once your approval has been established your plan will automatically be
brought up to date and the coverage will be effective from that time forward. You will receive notice in writing from the Insurance Company.
