Life Insurance provides a lump-sum payment to the named beneficiary in the event the insured employee, spouse, or child dies.
Each year, at annual enrolment, you can increase your optional employee life insurance by $10,000 without completing a Statement of Health/Evidence
of Insurability (EOI) form.
If you choose to purchase additional units of optional employee or spousal life insurance, you must complete a Statement of Health/Evidence
of Insurability (EOI) form to provide evidence of good health. The insurer must approve the additional coverage before it will take effect.
The Statement of Health/Evidence of Insurability (EOI) form will be presented after you confirm your enrolment, or you can find it on the Home Page,
in your EOI folder.
Evidence of good health is not required for optional child life insurance. You can cover eligible dependent children once they are 24 hours old, and only
children who qualify as dependent children may be covered. The cost for child life insurance is the same regardless of the number of children
covered. Each child is insured for the same amount.
