Select Your Optional Life Insurance

In this benefit you can choose Optional Life insurance and complete your beneficiary choices. The cost of the coverage is calculated based on your age and smoking status. You are required to fill in a beneficiary(s) for this benefit.

Optional Employee Life insurance can be purchased to provide a benefit in addition to your Basic Life coverage.

Coverage may require you to provide evidence of your insurability (EOI).A tab will appear if EOI is required called [Pending]. At the end of the enrollment after you have click the «Confirm» button you will be provided with the required EOI forms for printing.

The form is required by the insurance company to determine whether you satisfy their health requirements.Once your approval has been established your plan will automatically be brought up to date and the coverage will be effective from that time forward. You will receive notice in writing from the insurance company.
