You may choose Optional Life insurance and complete your beneficiary choices. The cost of the coverage is calculated based on your age, gender and smoking status. You are required to designate one or more beneficiaries for this benefit. You may be required to provide evidence of your insurability (EOI).
A tab called "Pending" will appear if EOI is required. When you complete your enrollment and have selected « Confirm », the required EOI forms will be provided to you for printing.
This information is required by the insurance company to determine whether you satisfy their health requirements.
Once your approval has been confirmed, your benefits record will be brought up to date and the coverage will become effective at that time. You will receive a notice in writing from the insurance company. Detailed information is provided in the CEVA Benefits Guide located at the left of the page, under the « Info » tab.
