In this benefit you can choose Optional Spousal Life insurance. The cost of the coverage is rate calculated and based on your spouse's age, sex and smoking status. Your spouse may be required to provide evidence of your insurability (EOI) as indicated by the heading [Evidence Required].A tab will appear if EOI is required called [Pending]. At the end of the enrollment after you have hit the "Confirm" button you will be provided the required EOI forms for printing. The form is required by the insurance company to determine whether your spouse satisfies their health requirements.Once your spouse's approval has been established your plan will automatically be brought up to date and the coverage will be effective from that time forward. You will receive notice in writing from the insurance company.
