Spousal Optional Life Insurance

Under our Optional Insurance plans, you may purchase Life insurance for your spouse. You decide whether to take this coverage and how much is appropriate.

You may increase or decrease any Spousal Life option coverage at re-enrollment. Any increases in coverage or new application will require completion of a "Evidence of Insurability (EOI)" form.
