Employee Optional Life Insurance

Under our Optional Insurance plans, you may purchase additional amounts of Life Insurance for you. You decide whether to take this coverage and how much is appropriate.

You may increase or decrease any Optional Life option coverage at re-enrollment. Any increases in coverage or new application will require completion of a "Evidence of Insurability (EOI)" form.
