This page provides you with the opportunity to review the various dental coverage options, choose one and allocate flex dollars. If you click on the coverage descriptions underlined in blue you will be provided with a brief description about that option. Detailed information is provided in "Plan Details". If your spouse has coverage and you do not want an option you can click on the "Waive Coverage" button and provide information about your spouse's plan. If you wish to co-ordinate benefits with your spouse's plan, benefits paid will not exceed 100% of the eligible claim. The insurer has the right to receive and release information on benefits and, if necessary", collect any overpayments made by it.

80% basic preventative to a max of $2,00 per plan year; major restorative and child orthodontia - no coverage; exams 1 every 9 months (please refer to your benefits booklet for further details)

90% basic preventative to a max of $2,500 per plan year; major restorative and child orthodontia - no coverage; exams 1 every 9 months (please refer to your benefits booklet for further details)

This option has a two year lock-in.  100% basic preventative and 50% major restorative with a combined max of $2,500 per plan year; child orthodontia no coverage; 1 every 6 months exams (please refer to your benefits booklet for further details)
